What is Swing Bed?

A swing bed program may sound a little odd at first, but can be incredibly useful under the right circumstances. This program allows a patient to progress from acute care to a skilled nursing facility to rehab with skilled staff. All without leaving the hospital! After an injury or illness that requires hospitalization, it is sometimes necessary for further treatment before going home. Our swing bed program at Conejos County Hospital (CCH) allows for this transitional phase. While you are here, you will be cared for by our expert nursing and rehabilitation team in the hospital setting. Our goal in providing care during an extended hospital stay is to do all we can to assist you in getting back to your previous level of function and return home. Therapy will work with you 2-3 times a day to show improvements on your way to regaining your independence.
Who qualifies?

Patients with Medicare (we also accept some private insurances).

Patient must have a minimum of 3-night stay in an acute setting - inpatient status

Patient must require skilled nursing care or skilled therapy services

Patient must be referred and accepted within 30 days of the original hospital stay
What is covered?
For patients with Medicare:

20 days are covered in full
-
80 days with a co-payment
-
101+ not covered
For patients with Medicare advantage plans:

Insurance will approve a set number of days and provide us with a review date.

Once we have reached the review date, and the patient is still requiring a longer stay, updated notes will be sent to insurance and an extension or denial will be given.
Referral Process
Talk to your provider about this option. Inpatient hospital staff will send referrals:
1. Send referral to CCH Swing Bed Coordinator. Please include face sheet, history and physical, most recent progress note, therapy evaluations, and most recent therapy notes.
2. Once a referral is received, we will send it to our team for review and get back to you as soon as possible. Unless further information is needed, our goal is within 24 hours.
3. Regardless of acceptance or denial, you will be notified of the decision and why. If accepted, we will coordinate transfer with your team to ensure we are prepared to receive the patient.
