Please carefully read through the information below before setting up your user. Then click the button to get to the correct patient portal. Clinic Patient Portal
If you have recently been seen at one of the following San Luis Valley Health Physician Services clinics: Family Medicine, Orthopedics, Internal Medicine, Women's Health, Pediatrics, ENT, General Surgery, Cardiology, or Oncology, please visit the Clinic Patient Portal.
Hospital Patient Portal
April 1, 2018. San Luis Valley Health will begin using one Patient Portal called, WebView (called clinic patient portal above). As a user of San Luis Valley Health facilities and providers, your clinical information will now be available on one portal. What this means for the Relay Health Portal: As of March 31, 2018, San Luis Valley Health will use Relay Health only to display test results taken before March 31, 2018. You will be able to view these test results on the Relay Health portal until September 28, 2018. If you need help with your password, viewing, or downloading your records on Relay Health you will need to call Relay Health Customer Service Hotline at 866-735-2963 for assistance. If you have a lab test after March 31st: Results will not be sent to Relay Health. These results will be available on your clinic patient portal. You must register as a new user on WebView. To register on WebView: Contact 719-587-1392 and ask for assistance to register on WebView.
If you have recently been seen at San Luis Valley Health's Emergency Room, been admitted to the hospital, or had Laboratory or Radiology visits, access the Hospital Patient Portal below.
Please be aware that test results that are flagged abnormal (e.g. results that are out of the normal range), may be normal for you due to your medical condition. Your physician will contact you to discuss any test results that may be of concern to your medical condition.
The Patient Portals are NOT to be used for urgent or emergency situations. In the event of an emergency, call 911, or go directly to the emergency room.
