SLVH Care Coordination Receives Appreciation Award

On August 2, 2023, SLV Health’s Care Coordination team was recognized for their partnership with Center for Restorative Programs (CRP). Carey Deacon, Director of CRP, awarded the Care Coordination team at SLV Health with an award. “SLV Health has been really supportive, and we wouldn’t be able to do all that we do without the team here.” CRP helps patients with housing, budgeting, transportation, and rehab referrals. They also help with obtaining an I.D. or driver’s license, birth certificate, or social security card. Particularly, SLV Health has been working closely with CRP to assist in providing wraparound services to our Medicated Assisted Treatment (MAT) patients.Front row: Audrey Reich-Loy, Michelle Trujillo, Ariel Devora
