San Luis Valley Health Offers Patients Access to Free Patient Portal

As a friendly reminder, several months ago San Luis Valley Health launched a new Patient Portal that patients are able to access for free as part of their care at San Luis Valley Health.

What is a Patient Portal?

A patient portal is a secure, online website that gives patients convenient 24 hour access to view their medical records from anywhere with an internet connection. Using a secure username and password, patients can view health information such as:

  • Recent hospital and doctor visits
  • Lab results
  • Medications
  • Immunizations
  • Allergies

How can I access the Patient Portal?

If you have recently been seen at one of the following San Luis Valley Health Physician Services clinics: Family Practice, Orthopedics, Internal Medicine, Obstetrics, Pediatrics, ENT, General Surgery, Cardiology, Oncology; please register for the Clinical Patient Portal.

If you have been recently seen at San Luis Valley Health’s Emergency Room, been admitted to the hospital, or had Laboratory or Radiology visits, please register for the Hospital’s Patient Portal.
