High Performing Hospitals

The Health Research & Educational Trust, in partnership with the American Hospital Association and Hospital Engagement Network, has released a list of High Performing Hospitals that have attained a 20 percent reduction in their readmissions. Both hospitals of the San Luis Valley Health system, San Luis Valley Regional Medical Center and Conejos County Hospital, have been named to this list.
Medicare's cost for unplanned hospital readmissions exceeds $17 billion annually. Among the most important factors contributing to unplanned readmissions are uncoordinated care and ineffective care transitions. Addressing complex issues across care settings is challenging, and requires new tools, communication channels, and care processes.
To be included in the list of top hospitals, each organization had to achieve a 20% reduction in readmissions in at least one of the following measures:
- 30-Day All Cause Readmissions
- Heart Failure 30-Day All Cause Readmissions
- Pneumonia 30-Day All Cause Readmissions
- AMI 30-Day All Cause Readmissions
