Community Health Needs Assessment
The public is invited to attend the upcoming Community Health Needs Assessments to learn more about SLV Health's commitment to advance care and learn how SLVH participates in the Hospital Transformation Program. Your attendance at our meetings, either in person or virtually, and your interest in taking our survey and sharing it with others in the community will help us advance our goals and strategies.
April 2, 2025, at the SLVH Grizzly Education and Conference Center, 1921 Main Street, 4:00-5:50 p.m. Register here. The ultimate goal of a community health assessment is to develop strategies to address the community's health needs and identified issues. A variety of tools and processes may be used to conduct a community health assessment; the essential ingredients are community engagement and collaborative participation.
